While we may not be designated providers with your health benefits plan, our staff will coordinate your care with your carrier when you choose to have your surgery performed by our doctors. Please call if you have any questions or concerns regarding your initial visit.
Please bring all medical, dental and other insurance information with you to the consultation so that we can expedite your care.
Cancellation/No Show policy for appointments
We understand that there are times when you must cancel an appointment due to work, family obligations or an emergency. However, when you do not call to cancel an appointment, you may be preventing another patient from getting a much needed appointment. Conversely, the situation may arise where another patient fails to cancel their appointment and we are unable to schedule you for an appointment you have been waiting for. Some larger appointments will require a deposit of $50.00 at time of scheduling. We require a minimum of 24 hours notice or your deposit will become non-refundable. We require you to speak to our receptionist on the phone. No messages left on service or a voicemail will allow a refund of your deposit.
We understand that delays can happen however we do our best to stay on schedule for all our appointment times. Your time is just as valuable to us as the Doctor’s time.
If a patient is 15 minutes past their scheduled time we will have to reschedule the appointment.

All of your information is considered confidential. We will ask that you sign a copy of HIPAA compliance form for our records. If you have any specific concerns about the privacy of your health records our doctors will be glad to address those concerns with you at any time.
